But to do this, you need to check to see your
fasting blood sugar (glucose) levels are 4 - 7 mmol/l (72-126 mg/dl) (when you wake up)
test your blood glucose levels before meals between, should be  4 and 7 mmol/l (72-126 mg/dl).
Remember, you still need tablets if you are ill;
if you are being sick or cannot swallow the tablets, |let your doctor or nurse know.
Occasionally test after meals (preferred levels less than 10 mmol/l) (180 mg/dl )

To achieve very good control (HbA1c 6.5-7.0%) you need:
.
a fasting pre-breakfast glucose less than 5.5 mmol/l (99 mg/dl )
.
pre-meal levels at other times less than 6.0 mmol/l (108 mg/dl )
.
after-meal levels (2 hours after a meal) less than 8.0 mmol/l (141 mg/dl).

These levels cannot be achieved in all patients. But if you are well and are prepared to stick to a healthy
diet and exercise your medication should be adjusted to achieve these levels, even if that means starting insulin.

How to Tell if you have Prediabetes
There are two different tests to see if you have normal blood sugar (glucose) or pre-diabetes:

1) the fasting plasma glucose test (FPG) or
2) the oral glucose tolerance test (OGTT).

If  you are pre-diabetic or diabetic, among the first issues you will have to deal with are blood sugar levels, irregular blood sugar levels, and just how to monitor your blood sugar level.

A basic blood sugar chart will give you an idea of what values you should be aiming for to take care of your health and stay clear of harmful health effects due to type 2 diabetes:

  •  Normal Fasting blood sugar (glucose) levels are 4 – 5.5mmol/l (70- 100 mg/dl) (when you wake up);  
       these amounts are typically less in the morning, and rise soon after meals.
     
  • Regardless of what you may have previously eaten, a random reading of 10 mmol/l (or 180 mg/dl) or more, may suggest you have type 2 diabetes.
     
  • A fasting blood sugar reading taken, for example, any time after you wake up each morning, should be, as mentioned  4 – 5.5mmol/l (70- 100 mg/dl).  

If the fasting blood sugar reading is 7 mmol/l (126 mg/dL) or more, you may have type 2 diabetes.

When monitoring your blood glucose levels, its crucial that you study any patterns with your readings with your doctor or diabetes nurse or healthcare professional, and take note of variations with meals, medications or other activity (like stress) that trigger undesirable readings.

Diabetes really is a serious problem, and can have devastating effects on  the whole body, on eyes, kidneys, heart, nerves, possibly leading to blindness, kindey failure, amputation of feet, heart illnesses and stroke. Those who have diabetes, regardless if it is type 1 or 2 is a real threat to health.

 

For those that have pre-diabetes it's by no means too early to consider aggressive preventive measures by altering your way of life and lifestyle choices.

    Type 2 Diabetes - Sugary Drinks OK ?

     

    Sugary drinks should not be a common menu option if you have type 2 diabetes. Should you remove them all together? Only you and your doctor can determine that. However, there are few benefits of keeping them in your Diet. Most sugary drinks contain nothing of significant nutritional value. The main thing they contain is sugar in various forms. When you drink a can of soda or sweet tea, the body quickly turns it into glucose quickly. It leads to spikes in glucose levels. If you need the sugar spike, think of drinking a fruit juice instead. If you want caffeine, switch to plain coffee or tea. There are many options for getting a tasty drink without ingesting sugary drinks.

     

    If you drink sugary drinks at least once a day, your chances of developing type 2 Diabetes is double that of those that don’t drink them that often. They contribute to weight issues and that leads to insulin resistance. For those with diabetes already, ingesting these drinks means they are less likely to be able manage their blood  glucose levels. Research links sugary drinks with elevated risks of heart disease and weakened bone structure. All of these show why these drinks are not a good choice for Diabetics.

     

    If you don’t think that sugary drinks are so bad, let’s look at the facts about some of them. A 12-ounce (340 mL) can of Coke contains 140 calories. It also has 39 grams of sugar.     Type 2 Diabetes - What Should I Look Out For ?

     

    Once you get a diagnosis of type 2 diabetes, what should you keep in mind? The disease comes with very serious complications. They can vary based on age, gender, and family origins. That is why you need to monitor for potential complications no matter who you are.  Family origins mean certain populations may have a higher chance of developing certain complications. Those of African American descent have a greater chance of getting the disease than others do. Adults in this group have a one in seven chance of developing the disease. By age, fifty-five, one out of four women in this group have it. Blindness is a common complication for that group along with amputation and renal failure.

     

    Native Americans have worse results than those of African American descent. By the time they reach retirement age, more of them will have the disease. Three out of ten in that population likely has pre-Diabetes at any time. People from Southeast Asia and the Pacific Islands get type 2 diabetes more often even if they are not obese. With obesity, the chances grow quickly. Add a family history or a couch potato lifestyle and it goes to the stratosphere. Looking into your family’s ethnic background can help you identify potential Type 2 Diabetes complications to monitor.

     

    Gender also plays a part in determining complications to monitor. Women can develop gestational diabetes during pregnancy. That can lead to losing the baby. Some babies develop birth defects due to the increase in blood  glucose. It also is a pre-cursor to developing type 2 diabetes. Women from non-white backgrounds have a higher chance of getting it than women from white backgrounds. Complications such as eating disorders and depression are higher in women as well. Men get heart  attacks and strokes more often than women do. The glucose levels often attack their kidneys sooner than in women. Vision problems, blindness, and gum disease often strike men as well.

     

    So what should you watch for with type 2 diabetes? All complications can hit anyone that has the disease. Here is a list of potential complications: vision problems, blindness, gum disease, heart attacks, heart disease, eating disorders, depression, weight gain, weight loss, excessive thirst, nerve damage, renal failure, circulation problems, and amputation. If none of those is bad enough, Diabetic coma is a severe complication. The final one is death. Which one do you want to experience? If you know better, the answer is none.

       Managing Type 2 Diabetes

    Type 2 Diabetes - Other Health Problems Too !

     

    You just received a diagnosis of type 2 Diabetes. In addition, if that weren’t bad enough, your doctor may tell you that you are at risk of developing other major conditions that could be life threatening. Finding out what those might be is one way to take some control over the situation. Here are some of the big ones. With diabetes, you have an increased risk of heart disease, stroke, depression, nerve damage, renal failure, amputation, and blindness. You can prevent or significantly delay all of these if you take proactive measures early.

     

    Heart disease is one of the major concerns. High levels of glucose cause damages to blood vessels all over the body. People with type 2 diabetes also appear to get cholesterol easier than others do. They also have issues with high blood pressure. All of this can lead up to a heart  attack or even a stroke. It is extremely important to keep a strict eye on your heart health. Taking measures to eat a healthy Diet and to exercise are good ways to be proactive with this. Losing weight is even better though.

     

    Elevated glucose levels cause blood  vessels around the body to constrict more than normal. They can also damage nerves. In the feet and legs, constricted blood vessels and nerve damage can lead to amputation. Nerve damage usually begins the problems. You start to lose feeling in the feet and lower legs. Then, you get a small wound, but don’t know it. The constricted blood flow slows down healing of the wound. In some cases, it may become infected. If the infection gets out of control, it can damage the tissue in this area of the body. This may happen a few times. Things may get to the point where your doctor will recommend an amputation to prevent further problems.

     

    Elevated glucose levels also affect the fluids in the eyes. It can damage retinal tissue. When this happens, loss of eyesight is common. The same damage may occur with the tissue of the kidneys. If enough damage happens, kidney failure can occur. Depression symptoms can come because of low blood  sugar. However, many people get the psychological form as well. Dealing with a chronic disease can be taxing to people. The worry can wear down people’s resolve. All of these show why it is so important to get control of your Type 2 Diabetes early. You can delay or even prevent any of them from happening. Apathy is the worst enemy.

       Preventing Type 2 Diabetes

    Type 2 Diabetes - What is Borderline Diabetes ?

     

    Borderline diabetes is an older term for what many now call pre-diabetes. This condition is a prelude to full-blown type 2 diabetes. You are literally on the borderline before crossing into the diabetes territory. You have elevated levels of blood glucose but have not reach the point of a diabetes diagnosis. Diabetes is a progressive disease. From its inception, blood glucose begins to rise. Because it is often slow to manifest, it may take years before diabetes develops. You can actually look at getting a diagnosis of pre-diabetes as a good thing. It is a wakeup call. You need to start taking preventative measures. It is also a wakeup call to prevent the other medical complications that accompany diabetes.

     

    In some people, stopping the journey into diabetes is impossible with today’s treatment options. However, for a lucky few, it is possible. The results depend on your overall health and other factors. Taking preventative steps early benefits everyone. That is true even for those that later develop full Type 2 Diabetes. Even making moderate changes will slow down or reverse pre-diabetes. Common recommendations for those with borderline Diabetes is losing five to ten percent of current body weight and exercising 30 minutes a day, five days each week. They can have a large impact on the future.

     

    Modest changes give you a fighting chance. However, changes that are more radical increase the chances of pulling back from borderline diabetes. Slimming down to your ideal weight is a good one to accomplish. Getting more exercise is another. Insulin resistance will go down with those two actions. Your blood  glucose levels will begin to normalize. Decreasing the amount of insulin resistance by any amount is progress in the right direction. With type 2 diabetes, normalizing blood glucose is a major accomplishment.

     

    Another important way to pull back from borderline diabetes is monitoring your blood glucose levels. Since you don’t have full type 2 diabetes, you likely don’t need a glucose meter. However, it is important to get diabetes screening at least once a year. It will help you see if the disease is progressing or pulling back. Regular screening will give you and your doctor data to determine what other steps you may need to take. Information is your friend when dealing with any aspect of Diabetes. If you can take enough action, you might be able to avoid falling prey to this life-threatening disease.

    Why is it so important for someone with type 2 diabetes to get healthy and active? Diabetes brings so many complications with it. You must constantly monitor your blood  glucose levels. Sometimes you might need to take medications or insulin to get things under control. The disease brings an increased threat of heart disease and stroke. You and your family need to make adjustment for the new realities of the disease. In addition, all of this happens in the midst of dealing with everyday life and normal obligations. All of these reasons sound good for skipping the part about getting healthy and active. Nevertheless, these very reasons are why you should.

     

    Losing weight is extremely important for someone with type 2 diabetes. Excess weight builds insulin resistance. If you are a couch potato, your choice to remain stationary also contributes to that resistance. Poor food choices high in carbohydrates don’t help either.     Type 2 Diabetes - Sugary Drinks OK ?

     

    Sugary drinks should not be a common menu option if you have type 2 diabetes. Should you remove them all together? Only you and your doctor can determine that. However, there are few benefits of keeping them in your Diet. Most sugary drinks contain nothing of significant nutritional value. The main thing they contain is sugar in various forms. When you drink a can of soda or sweet tea, the body quickly turns it into glucose quickly. It leads to spikes in glucose levels. If you need the sugar spike, think of drinking a fruit juice instead. If you want caffeine, switch to plain coffee or tea. There are many options for getting a tasty drink without ingesting sugary drinks.

     

    If you drink sugary drinks at least once a day, your chances of developing type 2 Diabetes is double that of those that don’t drink them that often. They contribute to weight issues and that leads to insulin resistance. For those with diabetes already, ingesting these drinks means they are less likely to be able manage their blood  glucose levels. Research links sugary drinks with elevated risks of heart disease and weakened bone structure. All of these show why these drinks are not a good choice for Diabetics.

    Type 2 Diabetes - What Should I Look Out For ?

     

    Once you get a diagnosis of type 2 diabetes, what should you keep in mind? The disease comes with very serious complications. They can vary based on age, gender, and family origins. That is why you need to monitor for potential complications no matter who you are.  Family origins mean certain populations may have a higher chance of developing certain complications. Those of African American descent have a greater chance of getting the disease than others do. Adults in this group have a one in seven chance of developing the disease. By age, fifty-five, one out of four women in this group have it. Blindness is a common complication for that group along with amputation and renal failure.

     

    Native Americans have worse results than those of African American descent. By the time they reach retirement age, more of them will have the disease. Three out of ten in that population likely has pre-Diabetes at any time. People from Southeast Asia and the Pacific Islands get type 2 diabetes more often even if they are not obese. With obesity, the chances grow quickly. Add a family history or a couch potato lifestyle and it goes to the stratosphere. Looking into your family’s ethnic background can help you identify potential Type 2 Diabetes complications to monitor.

     

    Gender also plays a part in determining complications to monitor. Women can develop gestational diabetes during pregnancy. That can lead to losing the baby. Some babies develop birth defects due to the increase in blood  glucose. It also is a pre-cursor to developing type 2 diabetes. Women from non-white backgrounds have a higher chance of getting it than women from white backgrounds. Complications such as eating disorders and depression are higher in women as well. Men get heart  attacks and strokes more often than women do. The glucose levels often attack their kidneys sooner than in women. Vision problems, blindness, and gum disease often strike men as well.

     

    So what should you watch for with type 2 diabetes? All complications can hit anyone that has the disease. Here is a list of potential complications: vision problems, blindness, gum disease, heart attacks, heart disease, eating disorders, depression, weight gain, weight loss, excessive thirst, nerve damage, renal failure, circulation problems, and amputation. If none of those is bad enough, Diabetic coma is a severe complication. The final one is death. Which one do you want to experience? If you know better, the answer is none.

With diabetes, you have an increased risk of heart disease, stroke, depression, nerve damage, renal failure, amputation, and blindness. You can prevent or significantly delay all of these if you take proactive measures early.

     

    Heart disease is one of the major concerns. High levels of glucose cause damages to blood vessels all over the body. People with type 2 diabetes also appear to get cholesterol easier than others do. They also have issues with high blood pressure. All of this can lead up to a heart  attack or even a stroke. It is extremely important to keep a strict eye on your heart health. Taking measures to eat a healthy Diet and to exercise are good ways to be proactive with this.     Elevated glucose levels cause blood  vessels around the body to constrict more than normal. They can also damage nerves.     Elevated glucose levels also affect the fluids in the eyes. It can damage retinal tissue. When this happens, loss of eyesight is common. The same damage may occur with the tissue of the kidneys. If enough damage happens, kidney failure can occur. Depression symptoms can come because of low blood  sugar. However, many people get the psychological form as well. Dealing with a chronic disease can be taxing to people. The worry can wear down people’s resolve. All of these show why it is so important to get control of your Type 2 Diabetes early. You can delay or even prevent any of them from happening. Apathy is the worst enemy.

    Borderline diabetes is an older term for what many now call pre-diabetes. This condition is a prelude to full-blown type 2 diabetes. You are literally on the borderline before crossing into the diabetes territory. You have elevated levels of blood glucose but have not reach the point of a diabetes diagnosis. Diabetes is a progressive disease. From its inception, blood glucose begins to rise. Because it is often slow to manifest, it may take years before diabetes develops. You can actually look at getting a diagnosis of pre-diabetes as a good thing. It is a wakeup call. You need to start taking preventative measures. It is also a wakeup call to prevent the other medical complications that accompany diabetes.

     

    In some people, stopping the journey into diabetes is impossible with today’s treatment options. However, for a lucky few, it is possible. The results depend on your overall health and other factors. Taking preventative steps early benefits everyone. That is true even for those that later develop full Type 2 Diabetes.     Twenty-five years ago, all we had was insulin and sulfonylureas. Consequently, we put patients
    on sulfonylureas and when they failed over time, they all ended up on insulin. Today we have
    many more medications to choose from. These medications work differently and they target
     the multiple pathophysiologic defects that we have in type 2 diabetes.

    We understand that behind all of this there is the insulin-resistant liver overproducing glucose.
    We have insulin resistance in the adipocytes.
    We have high free fatty acids, worsening insulin resistance, and perhaps worsening beta-cell function.
    We have insulin resistance in
     skeletal muscles as well. All of these defects are coming into play, as well as declining insulin
    over time because of progressive apoptosis or
     programmed cell death of beta cells."

    What is happening in Diabetes 2

    First, there is a shortage of insulin

    Second, there is insulin resistance.

    Third, there are genes

    These factors combine to cause Diabetes 2

     

    Pattern of progression

    1. At the beginning of Diabetes 2 a healthy diet may be sufficient to lower the sugar and keep the HbA1c below 7%
    2. Later, metformin is needed.
    3. Later still, add Exanatide if overweight or other drug.
    4. Later still insulin may be required

     

    Testing you sugar/glucose level
     

    See testing. If you 'embrace' your diabetes, you will gradually learn to control it and achieve an
    HbA1c of 7% or below.
    But to do this, you need to check to see your

    • fasting blood sugar  levels are 4 - 7 mmol/l (72-126 mg/dl) (when you wake up)
    • test your blood sugar  levels before meals between 4 and 7 mmol/l (72-126 mg/dl)
    • remember, you still need tablets if you are ill; if you are being sick or cannot swallow the tablets,
      |let your doctor or nurse know.
    • occasionally test after meals (preferred levels less than 10 mmol/l) (180 mg/dl )
    • To achieve very good control (HbA1c 6.5-7.0%) you need a fasting pre-breakfast glucose
      less than 5.5 mmol/l (99 mg/dl ), pre-meal levels at other times less than 6.0 mmol/l (108 mg/dl )
      and after-meal levels (2 hours after a meal) less than 8.0 mmol/l (141 mg/dl).
    • These levels cannot be achieved in all patients..but if you are well and are prepared to stick to a healthy
      diet and exercise your medication should be adjusted to achieve these levels, even if that
      means starting insulin.
